Community PartnersWhat are Community Partners? As the hub of the wheel for regional health care delivery, CVMC works closely with other groups based in and around the region to provide a full array of health services to central Vermonters. Some of our community partners include:
Central Vermont Home Health and Hospice CVHHH is a not-for-profit Visiting Nurse Association that belongs to and is responsive to the Central Vermont community. We provide comprehensive, high-quality home health and hospice care to all Central Vermonters, regardless of their illness, diagnosis, geographic remoteness, or ability to pay. In addition, the agency promotes the general welfare of the citizens of Central Vermont with health promotion and long term care services. Contact: (802)223-1878
Washington County Mental Health Services Washington County Mental Health Services (WCMHS) Inc. provides comprehensive, community-based services to adults with serious and persistent mental illness; developmental disabilities, mental retardation and autism; children with severe emotional disturbances and their families; children and adults with acute behavioral problems and a wide range of other challenging behaviors. People's Health and Wellness Clinic The People's Health and Wellness Clinic, located at 553 N. Main Street in Barre, VT provides care to those who might otherwise go without for lack of funds. The Clinic offers medical exams, primary care services, screenings, counseling and referrals. The clinic primarily serves patients at 200% of the poverty level who do not have other resources. (For example, an individual making $20,948 or less would qualify as would a family of four making $42,413 or less). People with Medicaid, Medicare, and VHAP do not qualify for treatment at the clinic, though they might be eligible for alternative care for specific conditions. Green Mountain United Way Green Mountain United Way programs provide services in central Vermont and the Northeast Kingdom for: - Children and their families
- Elderly
- Physically disabled
- Mentally handicapped
- Homeless
- Victims of violence and abuse
